The answer is C - Arteriography
Weight loss, fear of eating, and postprandial pain is most consistent with chronic mesenteric ischemia (intestinal angina). Because symptoms of insufficiency are related to increased circulatory needs during digestion of a full meal, little is likely to be seen in tests such as a small-bowel series or small-bowel biopsy. CAT scan is unlikely to be diagnostic since calcifications in the aorta or mesenteric vessels are not specific. The D-xylose test would also add little to any specific diagnosis and may well be normal. Arteriography, in this case, would be the most diagnostic and allow for appropriate surgical therapy.
