Dural Sinus Thrombosis - See a little discussion



A 21 yo woman was admitted with a right hemiparesis and global aphasia, after a brief
period of mental obtundation and behavioral disturbances. No fever was present, and
serum biochemical analysis was normal.

Discussion

A 21 yo woman was admitted with a right hemiparesis and global aphasia, after a brief period of mental obtundation and behavioral disturbances. No fever was present, and serum biochemical analysis was normal. MRI scan (left image, FLAIR sequence) showed marked swelling of the basal ganglia surrounded by edema.

T1 weighted, gadolinium-enhanced scan (middle) demonstrates the empty delta sign - absence of contrast in the superior saggital sinus. Dilated superficial cortical vessels can be seen at the right. Dural sinus thrombosis was diagnosed, and cumarinic therapy prescribed. The only risk factor detected was the use of hormonal contraceptives.
